Download the materials: PALS Online Score Entry and Reporting System Licensing Agreement renewal Contact Information Form Account Information Form       For PreK programs       For K-3 schools 2011-2012 Screening Calendar 2012-2013 Screening Calendar Managing Your PALS Online Account Step 2. Complete the Licensing Agreement renewal, Contact Information form, and Account Information form, and return them to the PALS Office via email (pals-help@virginia.edu), fax (434-982-2793} or mail. PALS Office University of Virginia PO Box 800785 Charlottesville, VA 22908 Your PALS Online System account will be activated and made available for your use once all forms are received at the PALS Office. Please note that every summer we transition our Online System by extracting data and optimizing for the coming fall and new school year. Thus, it is important in the spring/summer to print off desired reports from the concluding school year. Payment: We accept purchase orders or checks. Credit cards are not accepted.
